Undergraduate Forms Heading link

Many forms are only available in hard copy. Below is a list of links to common forms available online. You may also wish to consult the Academic Advisor for more information about college-specific forms.

Note: Not every undergraduate form can be completed online. Some forms, such as those relating to the College of Liberal Arts and Sciences, require paperwork or an in-person visit with an advisor (for example, late dropping a course, submitting a petition, etc.).

Many forms are only available in hard copy. Below is a list of links to common forms available online. You may also wish to consult the Graduate College’s forms page.

Note: not every petition to the Graduate College can be completed online. Some petitions require extra paperwork (for example, retroactive registration revision). All forms requiring the signature of the DGS should be turned in the Graduate Studies Program Coordinator (not the DGS personally).

Consult the Graduate Studies Program Director if you are unsure where the form you need is located or how to file your form.

A note on the Committee Recommendation Form: it must be submitted at least three weeks before the defense or exams take place. Doctoral candidates will not be able to start the iThenticate process until the Committee Recommendation Form is submitted.
